Private foundations and regional funders continue to provide meaningful support for community health centers and safety net providers. Below is a current roundup of national and regional opportunities with approaching deadlines — along with a note on state-level RHTP funding, which, while federally sourced, is flowing through state grant processes that function much like non-federal awards.

National Foundation Opportunities

Allen Foundation Grant

Deadline: July 15, 2026 | Average Award: $35,000 | Eligibility: 501(c)(3) nonprofit organizations

The Allen Foundation supports nonprofit organizations focused on improving human nutrition through education, research, training, and hunger relief. Priority goes to programs supporting early nutritional education for mothers and children, educator training, and community-based health initiatives. Wells Fargo – Anna Fuller Fund Grant

Created in memory of Anna Fuller, this fund provides grants to organizations that alleviate suffering from cancer through research, treatment, care, and prevention and treatment education. Oregon Community Grant Program (Oregon)

Deadline: June 23, 2026 | Award Amount: $5,000–$40,000 Eligibility: Nonprofits, tribal governments and organizations, churches, and governmental entities in Oregon

This program from the Oregon Community Foundation provides flexible funding to nonprofits addressing pressing challenges across Oregon, with particular priority given to small rural, culturally specific, and culturally responsive organizations. It supports community-driven, sustainable solutions that promote belonging, reduce inequity, and strengthen communities statewide. Oregon-based FQHCs serving rural or culturally specific populations should take a close look — the emphasis on local leadership and community trust aligns well with the FQHC model.

Con Alma Foundation – Northern New Mexico Health Grants (New Mexico)

Deadline: June 29, 2026 | Award Amount: Unspecified Eligibility: 501(c)(3) nonprofit organizations in good standing; tax-exempt government agencies and educational or research institutions in Los Alamos, Rio Arriba, and northern Santa Fe counties

The Northern New Mexico Health Grants Group, a collaborative initiative supported by Con Alma Health Foundation and the Los Alamos Medical Center Hospital Auxiliary, funds organizations working to improve health and address unmet healthcare needs in targeted northern New Mexico counties. FQHCs operating in this region should explore their eligibility and whether current programs align with the funder's priorities.

Telligen Community Initiative Grants (Colorado, Illinois, Iowa, and Oklahoma)

LOI Deadline: September 3, 2026 | Award Amount: Up to $150,000 over 24 months Eligibility: 501(c)(3) nonprofits, accredited schools, and public/governmental agencies in Colorado, Illinois, Iowa, and Oklahoma — priority given to rural projects

Telligen Community Initiative provides grants to organizations addressing social determinants of health, with a focus on integrating childhood, family, and community well-being. Priority areas include improving access to culturally appropriate prenatal care, supporting healthy births, and strengthening parenting resilience to prevent or reduce adverse childhood experiences. The explicit priority for rural projects makes this a strong fit for FQHCs in these four states.
